End of Life Care

Palliative care is the total care of patients for whom cure is no longer possible and when control of pain, other symptoms, psychological, social and spiritual problems are paramount.  We see the patient and family as a unit of care and our aim is to enhance the quality of life of patients, as well as support for families facing the loss of a loved one.  We see the patient as a valued individual, preserving quality of life, dignity and independence as far as possible.

Our Aim is to:

  1. Answer all questions honestly.
  2. Plan care in partnership with the patient, their families and carers.
  3. Ensure a holistic approach to their care.
  4. Co-ordinate care with other agencies as appropriate eg Macmillan Service, Marie Curie, Evening Service, District Nurse and Occupational Therapist.
  5. Support patients’ preferences regarding the place of care, whether at home, hospital, hospice or nursing home and support this choice whenever possible.
  6. Not judge patients for the decisions they make which may be contrary to the advice or beliefs of others.
  7. Give patients and their families the information needed to participate in decisions about their care.
  8. Disseminating information within the team about the patient and their family/carer at our Palliative Care meetings.
  9. Access to GPs/District Nurses via home visits if unable to attend the surgery.

It is our hope at The Glen Medical Group that these aims will enable us to provide high standards of care and support to patients and their families with advanced malignant disease and other end stage diseases and the right to die with peace and dignity in the setting of their choice.

The Glen Medical Group has recently achieved a Kite Standard Award for Palliative and Cancer Services within the Practice.
